We’ll review the available evidence in detail, discuss each of the care domains and identify any important evidence gaps. Following the consultation, Dr Ros Setterfield will prepare a completed Decision Support Tool (DST) as an independent assessment and preparation tool, using the information discussed during the consultation and the supporting documentation provided. Practical CHC Support goes beyond discussing your circumstances. It includes detailed preparation and the production of practical working documents that you can use throughout the DST assessment process. Because this service builds directly on the work completed during your Practical CHC Support consultation, it is available only to clients who have completed that service. This ensures that your representative has a thorough understanding of your circumstances, the available evidence and the preparation already undertaken. Your representation package includes: ✓ Advance review of your completed Practical CHC Support documentation ✓ Final preparation before the assessment ✓ Attendance by Dr Ros Setterfield at your Decision Support Tool (DST) assessment ✓ Independent professional representation throughout the assessment ✓ Clarification of the available evidence and NHS Continuing Healthcare eligibility criteria where appropriate ✓ Written summary of the assessment together with practical recommendations and suggested next steps This optional service is available by prior agreement following completion of ARROW Practical CHC Support and can be purchased once representation and date has been agreed.
