2–3 hour Microsoft Teams consultation with advance case preparation Fee: £425 fixed fee The ARROW Practical CHC Support service provides dedicated one-to-one support to help you understand whether NHS Continuing Healthcare may be worth pursuing before deciding whether to begin the assessment process. We’ll review the information and relevant documents you provide, discuss the person’s health and care needs in detail and consider the available evidence against the NHS Continuing Healthcare eligibility criteria. Following the consultation, you will receive a bespoke written report exploring the relevant care needs and risks, with indicative Checklist levels aligned with the relevant Decision Support Tool (DST) descriptors, together with analysis of the Four Key Characteristics and Primary Health Need test, identification of important evidence gaps and practical recommendations about the most appropriate next steps. Practical CHC Support goes beyond a general discussion of your circumstances. It provides a detailed, case-specific assessment to help you make an informed decision about whether and how to pursue NHS Continuing Healthcare. Practical CHC Support can help with:
✓ Reviewing the available evidence against the NHS Continuing Healthcare eligibility criteria ✓ Discussing the person’s health and care needs in detail across each of the CHC care domains ✓ Identifying significant needs, risks and interactions between different areas of care ✓ Identifying indicative Checklist levels and considering how the person’s needs may align with the relevant Decision Support Tool (DST) descriptors ✓ Exploring the Four Key Characteristics of nature, intensity, complexity and unpredictability ✓ Considering whether the overall picture may indicate a Primary Health Need ✓ Identifying strengths, weaknesses and important gaps in the available evidence ✓ Developing a practical strategy and discussing whether, and how, NHS Continuing Healthcare should be pursued Any assessment provided by ARROW is an independent specialist assessment based on the information and evidence available. It is not an NHS Continuing Healthcare assessment, recommendation or funding decision. Your Practical CHC Support package includes:
✓ Advance review of the information and relevant documents you provide ✓ A dedicated 2–3 hour Microsoft Teams consultation ✓ Detailed assessment of the person’s health and care needs across the NHS Continuing Healthcare care domains ✓ Indicative Checklist levels aligned with the relevant Decision Support Tool (DST) descriptors ✓ Analysis of the Four Key Characteristics and Primary Health Need test ✓ Identification of important evidence gaps together with practical recommendations ✓ A bespoke written report setting out the assessment, findings and recommended next steps Optional CHC Assessment Representation Following completion of your Practical CHC Support consultation, you may choose to instruct Dr Ros Setterfield to attend your NHS Continuing Healthcare Checklist assessment as your independent representative. Because this service builds directly on the work completed during your Practical CHC Support consultation, it is available only to clients who have completed that service. This ensures that your representative has a thorough understanding of your circumstances, the available evidence and the preparation already undertaken. Your representation package includes: ✓ Advance review of your completed Practical CHC Support documentation ✓ Final preparation before the assessment ✓ Attendance by Dr Ros Setterfield at your NHS Continuing Healthcare Checklist assessment ✓ Independent professional representation throughout the assessment ✓ Clarification of the available evidence and NHS Continuing Healthcare eligibility criteria where appropriate ✓ Written summary of the assessment together with practical recommendations and suggested next steps This optional service is available by prior agreement following completion of ARROW Practical CHC Support and can be purchased once representation and the assessment date have been agreed. Fee: £1,000 fixed fee Prefer for your case to be fully managed for you?
If, following Practical CHC Support, you would prefer ARROW to take responsibility for preparing, progressing and representing your case through the NHS Continuing Healthcare assessment process, Comprehensive CHC Support may be available. This service is offered where we consider that there is a genuinely strong case to pursue and ARROW is able to accept the case for representation. The service covers case preparation and progression through the Checklist and, following a positive Checklist, the full Decision Support Tool (DST) assessment and eligibility decision. Fee: £5,000 fixed fee The £425 Practical CHC Support fee is deducted from the total fee where Comprehensive CHC Support is subsequently agreed.
