This service is for individuals beginning their NHS Continuing Healthcare (CHC) journey and preparing for a Checklist assessment using the NHS Checklist Tool. The Checklist is a crucial first step, acting as a screening tool to determine whether an individual qualifies for a full eligibility assessment. Ensuring it is completed accurately is essential—if needs are underassessed, the individual may be wrongly excluded from progressing to the next stage of the CHC process.
