​NHS Continuing Healthcare (CHC) is a package of care fully funded by the NHS for adults over the age of 18 in the UK, with significant ongoing health needs who are being looked after outside of hospital.

If you or your loved one requires 24-hour care and are currently self-funding or contributing to a local authority care package, you may be eligible for funding that covers 100% of care costs.
