Joint Packages of Care When Joint Funding is Needed In some cases, an individual’s health and social care needs are equally significant. If a person is not eligible for full NHS Continuing Healthcare (CHC) but has health needs beyond what the local authority can legally provide, funding may come from both the NHS and the local authority, creating a joint package of care. Eligibility for Joint Funding Joint funding applies when specific needs, identified through the Decision Support Tool (DST), exceed the local authority’s legal responsibilities under Sections 18–20 of the Care Act 2014. These packages can be provided in any care setting, including a private home or care facility. Collaboration Between NHS and Local Authorities Integrated Care Boards (ICBs) and local authorities must work together to define their responsibilities in joint care arrangements. Decisions should be based on the individual’s specific needs, with a partnership-based approach ensuring seamless care delivery. In cases of overlap, one party should take the lead in coordinating and commissioning care. Additional NHS Services in Joint Packages Alongside NHS-funded Nursing Care, individuals may receive additional healthcare services, such as: • Primary healthcare (e.g., GP visits) • Medical assessments by doctors and registered nurses • Rehabilitation, reablement, and recovery • Community health services • Respite healthcare • Specialist healthcare support • Palliative and end-of-life care Funding and Care Delivery Joint care funding may be structured through: • Direct services provided to the individual • Commissioned services tailored to care needs • Financial contributions between the NHS and local authority • Integrated personal budgets Care delivery can involve a single provider or multiple professionals, ensuring a holistic approach.
