Who is this service for?

This service is for individuals seeking to request an assessment of eligibility for a Previously Unassessed Period of Care (PUPoC). It applies in cases where there is evidence that the individual should have been assessed for NHS Continuing Healthcare (CHC) at the time but was not, and where they personally funded that care either partially or fully. What we do ✅ Perform Preliminary Checks We confirm that the care period(s) meet(s) eligibility criteria for review by the commissioning Integrated Care Board (ICB). ✅ Analyse Medical & Care Records We gather and examine all available records to identify key evidence supporting your claim for funding. ✅ Liaise with the ICB We submit the necessary authorisation to act on your behalf and formally request that the ICB consider the claim. ✅ Complete Documentation We prepare and submit the Appendix 1 application form, detailing the individual’s care needs and medical history for clarity. ✅ Submit Supporting Evidence We provide the completed Appendix 1 form with supporting medical records and care payment proof to assist the ICB in developing an accurate Needs Portrayal Document. ✅ Provide Additional Feedback Once the draft Needs Portrayal Document is shared, we review it and provide further evidence or feedback within the 28-day window to ensure an accurate representation of needs.
