Who is this service for?

This service is for individuals already receiving NHS Continuing Healthcare (CHC) and who are due for a review. CHC eligibility is reassessed to ensure the care plan remains appropriate for the individual’s needs. The first review typically takes place three months after the initial funding decision, followed by annual reviews to determine continued eligibility.
