The NHS Continuing Healthcare (CHC) assessment process involves three key tools used by health or social care professionals to standardize the evaluation of an individual’s health needs.

The Fast Track Pathway Tool is used when a person’s condition is rapidly deteriorating, typically nearing the end of life. The Checklist Tool serves as the initial screening stage, helping to determine if further assessment is needed. Finally, the Decision Support Tool is used during the full assessment, allowing professionals to document the individual’s health needs in a structured way.
