The NHS Continuing Healthcare Assessment Process

The NHS Continuing Healthcare (CHC) assessment process consists of three main assessment 'tools' which give the health or social care professionals carrying out the assessments a standardised format in which to record details about individual's health needs. These are:

1. The Fast Track Pathway Tool - this is used when an individual's condition is deteriorating rapidly and they are nearing end of life
2. The Checklist Tool - this is used as the initial screening stage of the assessment process
3. The Decision Support Tool - this is used in the full assessment and provides assessors with a standardised format on which to record the relevant details about an individual's health need
